Case study: Humber and North Yorkshire ICB
ICBs across the country are working to better understand their local communities in response to the NHS Ten Year Plan and SHAPE is supporting them in the creation of a true Neighbourhood Health Service.
Humber and North Yorkshire ICB needed to bring information together across their six Places to build a picture of which neighbourhoods were underserved or had the highest health needs. With population data stored in several places, they need a more accurate and efficient way of getting this insight to enable them to make informed decisions.
Better neighbourhood insights
SHAPE supported the ICB to develop a bespoke Neighbourhood Planning version of SHAPE, as part of their suite of population management tools.
Once neighbourhoods were defined across their six Places, SHAPE enabled them to explore each neighbourhood on a granular level. They used insights across a range of metrics, including demographic data such as population age bands, deprivation and ethnicity, to get an overview of each neighbourhood. They then cross referenced this with estates and service information, also mapped within SHAPE, to show any gaps in provision and under or over-utilised space.
Taking it a step further
Once the ICB had a good understanding of their individual neighbourhood make-ups, they could then look further into health and social care performance in each neighbourhood. By reviewing data in SHAPE such as A&E attendances, hospital admission rates and recorded prevalences of a number of health conditions, they can start to prioritise which neighbourhoods are suffering the worst health inequalities, and where specific services should be targeted to achieve the best result.
This gives Humber and North Yorkshire ICB a solid foundation to build upon when considering where to locate Neighbourhood Health Centres, and what services should be prioritised in individual communities.

The ability to overlay data such as population growth, deprivation, disease and age bands with the facilities that house services allows us to visually see whether they are in the right place to meet the needs of each community, now, and in years to come. We’re building upon the standard data available to include new neighbourhood boundaries together with aggregated patient and activity data, enabling us to pinpoint gaps in service provision and make changes that will have the biggest impact.”
