SHAPE Neighbourhood Health Planning Dashboard

Following the Government’s announcement to roll out the first neighbourhood health services in 43 places, SHAPE is helping ICBs identify the best locations for services to be delivered.

SHAPE’s Neighbourhood Health Planning Dashboard is specifically designed to support these decisions through identifying existing assets, population needs, travel and access to facilities.

The dashboard offers critical data insights for improving care for adults with multiple long-term conditions, tackling health inequalities, and building preventative, place-based care models, all core commitments of the NHS Long Term Plan.

Key features include:

  • Live integration with SHAPE datasets, ensuring always up-to-date insights
  • Unlimited users, enabling system-wide collaboration
  • Analysis at Place and ICB levels, aligned with local service transformation
  • Comparisons nationwide, to highlight priority areas

Population Health Metrics:

  • Prevalence of long-term conditions (e.g. cancer, CVD, dementia, asthma, mental health)
  • Projected increases in key age groups (0-4s and 65+)
  • Ethnic diversity, deprivation, unemployment, disability, and life expectancy
  • Smoking rates and mental health burden
  • Index of Multiple Deprivation and Core20PLUS5 indicators

Estates and Workforce Intelligence:

  • GP workforce capacity, including age breakdown
  • Registered patient counts
  • NHS estate data (e.g. CHP, NHS PS, CDCs) with vacant space
  • Accessibility analysis to determine population coverage within 30 minutes by public transport
