Shaping your Neighbourhood Health Implementation Plan

Last week the Department of Health and Social Care and NHS England invited Places and their ICBs to submit applications to become part of the first wave of the National Neighbourhood Health Implementation Plan. The initial focus for the first Places will be creating Neighbourhood Health systems and processes for adults with multiple long-term conditions and rising risk before progressing to other areas. The 8th August deadline for submissions is less than two weeks away.

Get a head start with illustrating why your Place should be in the first wave with data led evidence from SHAPE:

Illustrate the most recent population demographic information to get an understanding of target communities
